Most U.S. hospitals are 501(c)(3) nonprofits, which are legally required to maintain a written financial-assistance policy and make it widely known. Uninsured patients are often eligible for a substantial discount — sometimes full forgiveness — based on household income, but only if they know to ask.

Why this happens
Financial assistance policies exist, but hospitals aren’t always proactive about surfacing them — the burden typically falls on the patient to find and request the application before paying anything.
